1. One of the most important things for our Members to know that is if you have sick time that you would like to convert into the RHRA, the company needs a minimum of four months’ notice. Use the above button to access Jetnet to apply for the RHRA. 

2. It’s also best to notify the IAM NPF five to six months in advance of when you would like to start receiving benefits. Also please follow up with the IAM NPF every step of the way, we are hearing from some that they can be less then timely and are not the best at communicating the status of your request. See the NPF button above.

3. Fill out the CMS-L564 Request for Employment Information Form, send to AA, after receiving back from AA get it to the Social Security Administration. Be sure to follow up to every step, this process can also be less then timely.

WHAT IS THE PURPOSE OF FORM CMS-L564?

In order to apply for Medicare in a Special Enrollment Period, you must have or had group health plan coverage within the last 8 months through your or your spouse’s current employment. People with disabilities must have large group health plan coverage based on your, your spouse’s or a family member’s current employment.

This form is used for proof of group health care coverage based on current employment. This information is needed to process your Medicare enrollment application.

The employer that provides the group health plan coverage completes the information about your health care coverage and dates of employment.
